- Name of Person Responsible for Account
- Valid Mailing Address and Telephone Number
- Copy of Valid Driver's License
- Name of Home Owner and/or Renter
- Date of Birth
- Social Security Number
- Landlord Phone Number (if applicable)
- Current Employer Phone Number
- Name of all Occupants in the Home
- Lease agreement if renting
To open a new water account please print out the form linked below, provide all information requested on the form, and bring the completed form with your $150 deposit to the Clyde City Hall located at 222 Oak Street.
